Comments:
A renewal inspection was completed on 01/31/2023. There were 24 children present with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed. The inspection started at 3:30pm and concluded at 4:55pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-140-A
Description: Based on record review, the center failed to have a physical examination byor under the direction of a physician for children in care.
Evidence:
Five children's records were reviewed. All five records did not have a physical exam completed by a physician. There was a medical form completed by the parents in the file. The director verified physicals were not in the files.

Plan of Correction: The director will continue to work with school staff to get copies of the physicals for the files.

Standard #: 8VAC20-780-330-B
Description: Based on observation and measurement, the center did not meet the requirement of where playground equipment is provided, resilient surfacing shall comply with minimum safety standards.
Evidence:
The mulch under the swings measured between 1-3 inches. The required depth is 6 inches.

Plan of Correction: The director will notify other program staff the need for mulch.
