Comments:
An unannounced monitoring inspection was conducted today from 10:30am-12:30pm. There were 102 children (infants-5yrs.) supervised by 15 direct care staff. The physical plant, 2 new staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, small groups activities, playing outside, and eating lunch. Diapering and hand-washing procedures were also observed. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Standard #: 8VAC20-780-60-A-8
Description: Based on record review, a written care plan for each child diagnosed with a food allergy was not obtained.
Evidence:
1. Child #6 has a diagnosed food allergy and has emergency medication on site. Child #6's record did not contain a written care plan.

Plan of Correction: The parents have been notified and are working to get a written care plan.

Standard #: 8VAC20-780-240-A
Description: Based on record review and interview, staff did not complete the Virginia Department of Education sponsored orientation course within 90 calendar days of employment.
Evidence:
1. Staff #1's (Date of hire-10/10/22) record did contain documentation of the Virginia Department of Education orientation course.
2. Staff #3 confirmed that the orientation course had not been completed by Staff #1 to date.

Plan of Correction: The staff member will complete the Department's orientation course and the record will be documented.

Standard #: 8VAC20-780-270-A
Description: Based on observation, areas and equipment, inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence:
1. In the Sound Stage classroom, a pillow located in the library area of the classroom was torn and exposing the cushion inside.

Plan of Correction: The pillow was removed from the classroom at the time of the inspection.

Standard #: 8VAC20-780-510-H
Description: Based on review of medications, nonprescription medication was not in the original container with the direction label attached.
Evidence:
1. Child #7 has Allergy Relief medication on site. The medication was located in a plastic bag with the child's name on it and not in the original packaging.
2. Child #8 has Allergy Relief medication on site. The medication was located in a plastic bag with the child's name on it and not in the original packaging.

Plan of Correction: The original packaging for the non prescription medication will be obtained.

Standard #: 8VAC20-780-510-I
Description: Based on review of medications, one medication at the facility is not in the original container with a label attached.
Evidence:
1. Child #6 has an Auvi-Q auto injector at the facility. The medication is not in an original container with a prescription label attached.

Plan of Correction: The parents have been notified and the parents will get the original box and prescription label for the Auvi-Q auto injector.
