Comments:
A renewal inspection was initiated on 2/7/2023 and concluded on 2/7/2023. There were 101 children directly supervised by 15 staff members. Children were observed outside on the playground, engaged in group activities to include manipulatives, puzzles and beading. The physical plant, 5 child records, 5 staff records, medications, evacuation drills, emergency supplies and policies were inspected. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation, areas and equipment of the center, outside, are not maintained in a clean, safe and operable condition.
Evidence: There is a house play structure that the wooden platform is broken with loose wooden planks and a broken hinge. There is a sharp broken piece of wood on the floor of this structure. Children were observed playing on the structure.

Plan of Correction: The play structure will be removed from the playground immediately.

Standard #: 8VAC20-780-330-B
Description: Based on observation, resilient surfacing shall comply with minimum safety standards to create a fall zone free of hazardous obstacles.
Evidence: The inspector observed a child using the 2nd swing from the right side of the swing set.. A very large piece of concrete and a wooden beam was sticking out of the mulch directly under the swing.

Plan of Correction: Staff will be told that children are not allowed to use that swing, the concrete and wooden beam will be removed from under the swing and proper surfacing will be maintained.

Standard #: 8VAC20-780-350-B-1
Description: Based on observation, the staff to children ratio of 1 staff to 4 children in the infant room was not being followed.
Evidence: On 2/7/2023 at 10:50 am there were 14 infants observed with 3 staff members. A fourth staff member arrived to the classroom at 11:15 am.

Plan of Correction: Ratios will be maintained at all times.

Standard #: 8VAC20-780-350-B-4
Description: Based on observation, the staff to children ratio of 1 staff to 10 children in the older fours program was not being followed.
Evidence: On 2/7/2023 at 10:30 am there were 18 children ages 4 years old observed outside on the playground with 2 staff members. Around 10:35am 1 staff member brought 1 child inside to use the restroom, leaving 17 children outside with 1 staff member.

Plan of Correction: Staff will take enough children inside to maintain ratios at all times or staff will call for assistance when a child needs to use the restroom.

Standard #: 8VAC20-780-510-E
Description: Based on a review of medications, the center is not following proper procedures for administering long term medication to include written authorization from both physician and parent to administer medication.
Evidence: Child #1 has an emergency long term medication, There is no written authorization from the parent to administer this medication.

Plan of Correction: Management will have the parent complete the written authorization for the medication.

Standard #: 8VAC20-820-120-A-4
Description: Based on observation and owner interview, the facility is not operating within the terms of the license, which includes the maximum number of children who may be in care at any one time.
Evidence: The current license allows for 99 children. During the inspection on 2/7/2023 there were 101 children in care.

Plan of Correction: Three children were drop in care this day and were not scheduled to attend. Administration will monitor drop in care to allow drop in care only when attendance allows. There are several children transferring to the sister center within the next couple weeks.
