Technical Assistance:
Crib spacing was discussed with the program director.

Comments:
A monitoring inspection was conducted on 02/07/2023. A tour of the facility was conducted, in addition to a record review, interviews with staff, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation the center did not ensure all areas and equipment of the center were maintained in a safe condition.
Evidence:
The kitchen located in the main hallway of the daycare, had the door propped wide open. There were 4 large chopping knives in the sink caddy.

Plan of Correction: The provider will begin to close the kitchen door when unattended in addition to drying the sharp knives and placing them in a locked area instead of on the drying rack.

Description: Based on observation the center did not ensure hazardous substances were kept in original containers, the substitute containers shall clearly indicate their content.
Evidence:
1. On the playground there was unlocked supply shed against the wall of the building. Inside there were 2 spray bottles containing a liquid substance, one yellow and the other green. Neither bottle wads labeled to identify the contents inside the spray bottles.

Plan of Correction: The director will ensure all substitute containers are labeled with its contents and will remind staff that chemicals are to be kept in a locked place.

Description: Based on observation the center did not ensure cribs met all requirements.

Evidence: In the infant room a baby was observed sleeping in a crib that did not have least 12 inches of space between the sides and ends of occupied cribs and Thirty inches of space between service sides of occupied cribs and other furniture where that space is the walkway for staff to gain access to any occupied crib. There was furniture in between a row of cribs.

Plan of Correction: The director will rearrange the infant room to ensure all cribs have the required spacing.
