Technical Assistance:
Technical Assistance given and Licensing Standards were reviewed regarding mounting fans, latching safety belts, and labels to staff personal items.

Comments:
An unannounced renewal inspection was conducted on February 7, 2023. The inspector arrived at 9:10am AM and departed at approximately 12:00PM. There were 44 children present and 8 staff members. A sample size of 12 records were reviewed which included six children?s records and six staff records. The Inspector observed staff and children?s interactions throughout the inspection. Children were observed during group activity, free play both indoor and outdoor, snack, and naptime. First aid kits and emergency preparedness documents were observed. Medications were observed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented. The violations are listed on the violation notice issued to the center and were reviewed with the site owner/director at the exit interview.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation and interview, it was determined that the center did not ensure that areas and equipment of the center, inside and outside, be maintained in a safe and operable condition.
Evidence:
1. In the toddler classroom, the blue shelf at the entrance of the classroom is chipping paint and wood to the corners and end of the shelving. This poses as an injury hazard to the children in care as they are able to reach and use the shelf.
2. Staff #1 confirmed that the blue shelf is chipping both paint and wood to the corners and end of the shelving and children are able to reach and use the shelf.

Plan of Correction: The center responded with the following: Staff #1 will have the book shelf repaired to prevent any splinters or injury to the children in care.

Standard #: 8VAC20-780-280-B
Description: Based on observation and interview, it was determined that the center did not ensure that hazardous substances such as cleaning materials be kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. In the toddler room, there were two containers of liquid floor cleaner with a warning label stating, ?Keep out of reach of children? on a shelf not locked to prevent access by children.
2. Staff #1 confirmed that the liquid floor cleaners were unlocked.

Plan of Correction: The center responded with the following: Staff #1 will secure all cleaning materials in a locked area to prevent access by children.

Standard #: 8VAC20-780-440-L
Description: Based on observation and interview, it was determined that the center did not ensure that pillows shall not be used by children under two years of age.
Evidence:
1. Two children in the infant room, under the age of two years old, were observed with pillows beside them during nap time in their designated sleeping area.
2. Staff #1 confirmed that pillows were being used in the infant room.

Plan of Correction: The center responded with the following: The pillows were immediately removed during the time of the inspection. Staff #1 will ensure that no pillows will be used by children under two years of age.

Standard #: 8VAC20-780-550-P
Description: Based on record review and interview, it was determined that the center did not ensure that they maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence including two signatures to the injury report by either a staff and/or parent.
Evidence:
1. Three of the five injury reports reviewed did not have two signatures to the injury report by either a staff and/or parent.
2. Staff #1 confirmed that the injury reports only have one signature in which two signatures are required.

Plan of Correction: The center responded with the following: Staff will ensure that the injury reports are filled out completely and have the two required signatures to the form. Staff #1 will review this form with staff at the next meeting.
