Comments:
A renewal inspection was conducted on 02/08/2023. A tour of the facility was conducted, in addition to a record review and interviews with staff. The inspector emailed the director/provider a list of items required to complete the inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-60-A
Description: Based on a record review the center did not ensure enrolled children had all the required information maintained in the record.
Evidence:
Child #1, did not have documentation of viewing proof of the child's identity and age.

Plan of Correction: This was an oversight. Admin staff will contact the parent for this information and document it in the record.

Standard #: 8VAC20-780-270-A
Description: Based on observation the center did not ensure all areas and equipment of the center, inside and outside were maintained in a safe condition.
Evidence:
1. On the playground there were 2 tricycles that had missing handlebar covers.
2. The concrete area of the playground hasd2 boarded-up doors that had peeling paint and splintered wood. There was also a rusted shovel next to this area.
3. In the nap room, the linoleum on the bathroom cabinetry was peeling and some of the cabinets were broken and off the hinge.

Plan of Correction: Admin has a list of physical plant needs that are being addressed one at a time due to financial constrictions. Many are in the process if being completed.

Standard #: 8VAC20-780-330-A
Description: Based on observation the center did not ensure where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99.

Evidence:
The slide on the playground had less than 3 inches of resilient surfacing where 6 inches is required.

Plan of Correction: There was a lot of rain recently and the mulch was pushed away from the equipment. The mulch will be redistributed and evaluated for additional mulch at that time.

Standard #: 8VAC20-780-330-C
Description: Based on observation the center did not ensure ground supports were covered with materials that protect children from injury.
Evidence:
On the playground the ground supports of the green climber with steering wheels on the playground were not covered with materials that protect children from injury.

Plan of Correction: There was a lot of rain recently and the mulch was pushed away from the equipment. Mulch will be redistributed to ensure the ground supports of this piece of equipment is covered in a way that protects children from injury.
