Comments:
A non-mandated monitoring inspection was conducted on February 6, 2023 from 9:00 A.M.-12:00 P.M. There were 55 children present, ranging in ages from two to five years of age, with 13 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of six child records and nine staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-40-E
Description: Based on review of medications and staff handbook, the center failed to follow their medication policy.

Evidence:
1. Child 1M had an emergency medication (Epi-Pen) on site. The epi-pen expired on 11/2022.
2. The faculty handbook states on page 12 under medication, "all medications must be checked regularly to ensure that they are within their expiration date."
3. Staff verified that the medication had expired.

Plan of Correction: We have modified our wording around checking the medication's expiration dates. We are currently working on the list that will be displayed in the office and checked. This will be implemented by next Friday, February 17th. A new Epipen has been provided by parents for the child. The expiration date is April 2024

Standard #: 8VAC20-780-240-A
Description: Based on review of staff records, the center failed to ensure that the Virginia Department of Education sponsored orientation course was completed within 90 calendar days of employment.

Evidence:
1. Staff #1's date of employment was 10/17/2022. There was not documentation in the file that the orientation course had been completed. It was required to be completed by 01/17/2023.
2. Staff 2's date of employment was 09/15/2022. There was not documentation in the file that the orientation course had been completed. It was required to be completed by 12/15/2022.
3. Administration verified that he orientation course had not been completed.

Plan of Correction: We will provide in service training for the staff noted by March 3rd

Standard #: 8VAC20-780-280-B
Description: Based on observation, the center failed to ensure that all hazardous materials shall be kept in a locked location.

Evidence:
1. In Classroom #7 on the shelf in the restroom there was a container of Microban 24 hour sanitizing spray.
2. The warning label stated, "keep out of the reach of children" and "caution".

Plan of Correction: We have notified cleaning staff to ensure no cleaning supplies are left behind. We have also notified the teachers to perform spot checks in the morning before children arrive to ensure no cleaning supplies are left within reach of the children. This has been implemented immediately.
