An unannounced monitoring inspection was conducted today between the hours of 10:15am-12:15pm. There were a total of 39 children with eight supervising staff members present. Children were observed during handwashing, engaged in group activities and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 8VAC20-780-240-A Description: Based on record review, the Virginia Department of Education sponsored orientation course was not completed within 90 calendar days of employment.
Evidence:
1. Staff # 1's (Date of hire-10/14/21) record did not contain documentation of the Virginia Department of Education sponsored orientation course at the time of the inspection.Plan of Correction: Staff previously completed the orientation course, but did not complete the final necessary steps to obtain a certificate. The staff member completed the necessary steps to finish the course and obtained a training certificate.
Standard #: 8VAC20-780-270-A Description: Based on observation, areas and equipment of the center, inside and outside, were not maintained in a clean, safe and operable condition.
Evidence:
1. In the Preschool 1 classroom, the plastic sink located in the dramatic play area was cracked and broken on the edge making sharp edges that could snag skin.
2. In the Pre-K classroom, the edge of the cushion of the sofa was worn and exposed the filling inside.Plan of Correction: The plastic sink was removed from the classroom at the time of the inspection and a new one was ordered. The cushion was taped for the time being and a new cushion will be ordered.
Standard #: 8VAC20-780-570-E Description: Based on observation, prepared infant formula was not dated.
Evidence:
1. Child #6 has three bottles of prepared infant formula labeled with their name and not the date.Plan of Correction: The parents have been informed about labeling the bottles with the child's name and date everyday. Administration put out notification to parents to ensure that all bottles are labeled with the name and date everyday.
