Comments:
An unannounced renewal inspection was conducted on 2/13/2023 from 10:00am-12:55pm. There were 101 children supervised by 15 direct care staff. The physical plant, 6 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in music, circle time, reading books, playing outside, and eating snack. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Areas of non-compliance are identified in the Violation Notice. Standard #: 8VAC20-780-40-K
Description: Based on record review and interview, the center did not develop written procedures for prevention of abusive head trauma.

Evidence: There was no documentation of a written policy and procedure for prevention of abusive head trauma.

Plan of Correction: All staff will be notified and trained.

Standard #: 8VAC20-780-280-B
Description: Based on observation and interview, hazardous substances such as cleaning materials, insecticides, and pesticides was not kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. In the butterflies classroom, two cans of air freshener were found on a shelf above the toilet in the bathroom.
2. In the hallway bathroom, next to the butterflies classroom, a can of disinfectant spray and toilet cleaner were found in a mirror cabinet above the sink.
3. In the Bears classroom, a container of disinfectant wipes was found on a wooden shelf.

Description: Based on record review and interview, the center did not develop a written policy and procedure that describes how the center will ensure that each group of children receives care by consistent staff or team of staff members.

Evidence: There was no documentation of a written policy and procedure that describes how the center will ensure that each group of children receives care by consistent staff.

Plan of Correction: All staff will be notified and trained.

Standard #: 8VAC20-780-510-L
Description: Based on observation and interview, the facility did not ensure that all medication was kept in a locked place, using a safe locking method that prevents access by children.

Evidence: observed that there was over the counter pain medications (ibuprofen and acetaminophen) inside a bathroom mirror cabinet, that was not locked.

Plan of Correction: Corrected during inspection.
