A monitoring inspection was initiated on February 13, 2023 and concluded on February 13, 2023. There were 25 children present with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program

Standard #: 22.1-289.035-B-4 Description: Based on record review and staff interview the center did not ensure to obtain a copy of the results of a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.
Evidence: Staff #1 confirmed that Staff #2?s first date of employment was 4-26-2022 and they had not obtain an out of state child abuse and neglect record check for Florida where they had lived within the last five years.Plan of Correction: Will send for out of State search for child abuse and neglect for staff.
Standard #: 8VAC20-780-160-A Description: Based on record review and staff interview the center did not ensure documentation of the Tuberculosis screening shall be submitted at the time of employment and prior to coming into contact with children.
Evidence: Staff #1 confirmed that Staff #4?s TB screening was dated 11-1-2022 and the first date of employment was 10-6-2022.Plan of Correction: Will make sure TB is done at time of employment.
Standard #: 8VAC20-780-40-M Description: Based on observation and staff interview, the licensee did not ensure the center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions. This list shall be dated and kept confidential in each room or area where children are present.
Evidence: Staff #1 confirmed that the list of children?s names listing their allergies and food sensitivities (in the 4-5 year old room) was dated 2021. She further stated additional information has been added since that time but she has not added the current date.Plan of Correction: Will add current date.
Standard #: 8VAC20-780-80-A Description: Based on observation and staff interviews the center did not ensure for each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.
Evidence: Staff #5, in the 2-3 year old room and Staff #4, in the 4-5 year old room both confirmed their attendance sheets were not accurate at the time of inspection.Plan of Correction: Staff will do attendance daily.
Standard #: 8VAC20-780-330-B Description: Based on observation and Staff interview the center did not ensure stationary equipment with climbing or moving parts on the playground had six feet on all sides of the equipment.
Evidence: Staff #1 confirmed the following items did not have six feet between them
1. There was approximately 3 feet between 2 caterpillar tunnels that the children climb through and on top of.
2. There was approximately 3 ? feet between a little tykes slide and the fence.
3. There was approximately 2 feet between a little tykes slide and the fence.Plan of Correction: Will rearrange pay area to meet guidelines.
Standard #: 8VAC20-780-440-E Description: Based on observation and staff interview the center did not ensure there shall be at least 12 inches of space between occupied cots, beds, and rest mats.
Evidence: Staff #1 confirmed there were only approximately 2-4 inches between 3 children in the 3 year old classroom and only 2-4 inches between 2 children in the 2 year old classroom.Plan of Correction: Will make sure adequate space is between mats.
Standard #: 8VAC20-780-450-A Description: Based on observation and Staff interview the center did not ensure Cribs, cots, mats and beds used by children other than infants during the designated rest period or during evening and overnight care shall have linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges. Cribs when being used by infants shall have a bottom cover.
Evidence: Staff #1 confirmed that one child in the 2-year-old room did not have a bottom cover and that one child in the 4-5 year old room did not have a top or bottom cover.Plan of Correction: Have asked parents to bring in correct covers.
