Comments:
An unannounced monitoring inspection was conducted on February 7, 2023 from 10:15 a.m. to 1:50 p.m. There were 111 children present, ranging in ages from four months to five years old with 24 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of nine child records and ten staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-B-4
Description: Based on review of records and interview, the center failed to ensure that out of state criminal history background checks were completed by the first day of employment.
1. Staff #7?s first day of employment was 08/22/2022. The sworn statement signed by Staff #7, indicated they had lived in another state within the past five years. There was no out of state criminal history background check in Staff #7's record.
2. Staff #8?s first day of employment on file was 12/22/2022. The sworn statement signed by Staff #7, indicated they had lived in two other states within the past five years. There was not an out of state criminal history background check in Staff #8?s file record for either state.
3. Staff #11 confirmed that the out of state criminal history background checks were not conducted for Staff #7 and Staff #8.

Plan of Correction: We requested all of these out of state documents directly following our inspection. We better understand the process now and will make sure to use the Interstate Childcare Background Check document to know how to request these out of state documents in the future.

Standard #: 8VAC20-780-560-G
Description: Based on observation and interview, the center failed to ensure that when food is brought from home, the contain is dated and labeled in a way that identifies the owner.
Evidence:
1. In Classroom #3, there were three water bottles that were not dated or labelled. Staff #11 confirmed the water bottles were not labelled or dated.
2. In Classroom #4, there were five lunchboxes that were not dated or labelled. Staff #11 confirmed the lunchboxes were not labelled or dated.
3. In Classroom #5, there were three water bottles and five lunchboxes that were not dated or labeled. Staff #11 confirmed that the water bottles and lunchboxes were not dated or labelled.

Plan of Correction: Staff members were reminded of the policy and required to communicate with their families using their classroom applications. An email also went to parents from the center office. Labeling stations were created at both of our entrances providing tape supplies for parents to use for daily labeling.
