Comments:
An unannounced monitoring inspection was conducted today between the hours of 9:15am-11:30am. There were a total of 63 children with 11 supervising staff members present. Children were observed during diapering, circle time, singing,engaged in group activities and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 8VAC20-780-160-A
Description: Based on record review, each staff member did not submit a completed tuberculosis screening within the last 30 calendar days of the date of employment.
Evidence:
1. Staff #2's record (Date of hire-10/11/2022) did not contain documentation of a tuberculosis screening dated within the last 30 days of the date of employment. The tuberculosis screening was dated 07/20/2022.

Plan of Correction: Moving forward, administration will ensure that a tuberculosis screening is 30 calendar days within the first date of employment.

Description: Based on record review, children's records did not have required information.
Evidence:
1. Child #2's record (Start date-10/31/2022) did not have documentation of a second emergency contact's name, address, and phone number.

Plan of Correction: Administration will obtain the information for a second emergency contact from the parents.

Standard #: 8VAC20-780-240-A
Description: Based on record review, staff did not complete the Virginia Department of Education-sponsored orientation course within 90 days of employment.
Evidence:
1. Staff #1's record (Date of hire-10/12/2022) did not contain documentation of a completed Virginia Department of Education sponsored orientation course within 90 days of employment.

Plan of Correction: The staff member will complete the VDOE sponsored orientation course.

Plan of Correction: Administration has printed out the Virginia Department of Education's model form for staff orientation. Each staff member will completed the staff orientation form.

Description: Based on observation, electrical outlets did not have protective covers.
Evidence:
1. In the Two's classroom, two electrical outlets were not covered with protective covers.
2. In the Pre-K classroom, two electrical outlets were not covered with protective covers.
3. In the Toddler classroom, two electrical outlets were not covered with protective covers.

Plan of Correction: Staff will be retrained to ensure that all electrical outlets are covered with protective covers.

Standard #: 8VAC20-780-510-I
Description: Based on medication review, the center did not maintain a prescription medication in the original labeled container.
Evidence:
1. Child #6's has albuterol on site. The albuterol was maintained in a plastic bag with the child's name on it, but the original box with the prescription label attached was missing.

Plan of Correction: The parents will be contacted to get a new container for the medication with a prescription label.

Description: Based on observation, food brought from home was not dated and labeled in a way that identifies the owner.
Evidence:
1. In the infant classroom, two containers of infant cereal were not labeled with Child #9's name.

Plan of Correction: All food from home will be labeled with the child's name and date.

Standard #: 8VAC20-780-570-E
Description: Based on observation, bottles were not labeled with the Child's name and date.
Evidence:
1. In the infant classroom, three bottles for Child #7 were not labeled with the child's name and date.
2. In the infant classroom, two bottles for Child #8 were not dated.

Plan of Correction: Staff will be retrained to ensure that bottles are labeled with names and dates.
