A renewal inspection was conducted on 2/13/2023 with center staff. Additional documentation was submitted 2/14/2023. There were 24 school age children present, ranging in ages from 6 years to 12 years, with 1 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 1 staff record were reviewed. The children were working on homework, playing a group game in the gym, and playing on the playground.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-140-A Description: Based on record review and staff interview, the facility failed to have documentation of a physical on file for each child within the first 30 days of attendance. Evidence: There was no physical on file for Child 2 and Child 3. Plan of Correction: Center staff will ask the parent to submit a physical for Child 2 and Child 3.
Standard #: 8VAC20-780-350-B-5 Description: Based on observation and staff interview, the facility failed to maintain the ratio of one staff for every 18 children ages 5 years to 9 years. Evidence: on 2/13/2023 from approximately 2:50 p.m. to 4:40 p.m., one staff was alone with 24 children. Staff 1 stated that there were no other center staff present. Plan of Correction: A second staff person arrived at approximately 4:40 p.m.
