Repeat of Phase 2 orientation class, maintenance of all documentation in one location in the facility.

Comments:
An investigation was begun on 1/25/23 in response to an allegation being received in our office.
Based on a review of the facility physical plant, review of staff and child records, and multiple staff interviews the preponderance of evidence supports the allegations. The complaint is valid.

Standard #: 8VAC20-770-60-B
Complaint related: Yes
Description: Based on a review of records, one staff record does not contain documentation of an accurate Sworn Disclosure Statement (SDS).
Evidence:
Staff A's SDS does not disclose all residences outside the state of Virginia.

Plan of Correction: Staff A will update the missing address on the sworn statement when they return from leave. This was an oversight, but Staff A was vetted through the standard new hire onboarding background check process to include VA state police/OBI. Action to take is to verify all addresses match background check forms and addresses listed on the sworn statement.

Description: Based on multiple staff interviews, it was determined that a child under 10 was not kept in actual sight and sound supervision.
Evidence:
An infant in the Ducklings room was not kept in actual sight and sound supervision when the child was sleeping in a crib in the classroom while the remainder of the class went outdoors.
A staff member inside the facility was supposed to be checking on the child every 15 minutes.

Plan of Correction: This was corrected in September 2022. Previously the rooms were connected by a small hallway where staff would check on both rooms via the connecting hall during naptime. After speaking with licensing regarding sight and sound supervision in September 2022, we added the door to separate the rooms. The rooms have been separated with a door since September 2022. Now a teacher is present in each rooms when children are present

Description: Based on direct observation, a ratio of 1 staff to 5 children was not maintained in the Toddler/2's room.
Evidence:
On 1/25/2023 the Bunnies room (ages 16 months to 24 months) was observed by the LI, out of ratio at 7:1.

Plan of Correction: Staff will be advised that they can ask for help and interrupt meeting with LI when LI are present to avoid ratio violations. This was corrected right away 1/25/23 while LI was present.

Description: Based on a review of documentation, the center did not maintain a written record of children's serious and minor injuries.
Evidence:
When asked for injury reports for specific children, the reports could not be located.

Plan of Correction: During the 1/25/23 inspection visit, LI requested to review the injury logs. While reviewing the logs, we discovered hard copies were missing. We believe the injury reports in question were taken out of the office. on 2/25/23 locks will be added to the office door. The office will remain locked at all times only accessed by Management or under management supervision. We are also implementing a new filing system.
