A renewal inspection was initiated on 02/07/2023 . There were 62 children present with 11 staff present. Additional staff were present to assist. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Records were reviewed remotely. A total of 4 child records and 6 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-160-A
Description: Based on record review, the licensee did not ensure staff had a negative tuberculosis screening been completed within the last 30 calendar days of the date of employment.


Staff #1 hire date 03/07/2022, most recent TB test was 03/11/2022.
Staff #2 hire date 10/03/2022, most recent TB test was 10/03/2022.
Staff #4 hire date 10/03/2022, most recent TB test was 10/22/2022.
Staff # 5 hire date 09/26/2022, most recent TB test was 09/26/2022.
Staff #6 hire date 10/17/2022, most recent TB test was 10/17/2022.

TB test results were not completed with in 30 days prior to hire.

Plan of Correction: We will make sure prior to the new employees start date that they have completed their TB testing.

Standard #: 8VAC20-780-500-B
Description: Based on observation and staff interview, the licensee did not ensure diapering surface be cleaned with soap and at least room temperature water and sanitized after each use.

Evidence: Staff confirmed they did not sanitize diapering surface between each use.

Plan of Correction: We will conduct a staff meeting and training with the staff on diaper changing. We will continue to monitor the staff during changing.

Description: Based on observation and staff interview, the licensee did not enusre there the menu be dated.


The menu posted did not have date. Confirmed by staff.

Plan of Correction: We will make sure each week that the menus are dated correctly.
