Comments:
A monitoring inspection was initiated on 02/15/2023 and concluded on 02/15/2023. There were 16 children present, ranging in ages from5 months to 4 years, with 7 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 6 child records and 6 staff records were reviewed.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Please complete the plan of correction and date to be corrected sections for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s): 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Violations:
Standard #: 8VAC20-780-140-A
Description: Based on record review and interview, it was determined the center did not ensure that each child had a physical examination by or under the direction of a physician within the required time frame.

Evidence:
1. The record for Child D (enrolled 09/09/2021) did not contain documentation of a physical exam.
2. The director confirmed this documentation was not available during this inspection.

Plan of Correction: I contacted the parent's and they will be getting the physical within the next day or two. I will be sure that we get the physical within the correct time frame.

Standard #: 8VAC20-780-160-A
Description: Based on record review, the center failed to ensure that documentation of a tuberculosis screening was obtained prior to staff coming into contact with children.

Evidence:
1. Staff # 5's tuberculosis test/screening was dated 12/06/2022. First day of employment and being in contact with children was 10/05/2022.

Plan of Correction: Not available online. Contact Inspector for more information.

Standard #: 8VAC20-780-550-E
Description: Based on record review, the center did not ensure to practice shelter-in-place procedures a minimum of twice per year.

Evidence:
1. The center had documentation showing that only one shelter-in-place drill was practiced during the year 2022.
2. The director confirmed that there was only one shelter-in-place drill that was practiced during 2022.

Plan of Correction: I will make sure that we complete two shelter in place drills annually.

Standard #: 8VAC20-780-550-F
Description: Based on record review, lockdown procedures were not practiced at least annually.

Evidence:
1. The center did not practice a lockdown drill in 2022.
2. The director confirmed that they did not conduct a lockdown drill in 2022.

Plan of Correction: I will be sure to complete a lockdown at least one time a year.
