Comments:
An unannounced renewal inspection was conducted today from 10:30am-12:35pm. There were 125 children (infants-5yrs.) directly supervised by 32 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play, circle time, show and tell, singing and counting activities, listening to stories and playing outside. There was an abundant supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified on the Violation Notice.

Description: Based on record review, the center did not obtain immunization documentation for a child prior their first day of attendance.

Evidence:
Child #4's record did not contain immunization documentation, the child's first day of attendance was documented as 10/17/2022.

Plan of Correction: Obtain immunization record from parent.

Standard #: 8VAC20-780-140-A
Description: Based on record review, the center did not obtain a physical examination for a child within 30 days of attendance.

Evidence:
Child #4's record did not contain a physical examination. The child's first day of attendance was documented as 10/17/2022.

Plan of Correction: Obtain physical exam from parent.

Standard #: 8VAC20-780-270-A
Description: Based on record review, areas of the center were not maintained in a safe condition.

Evidence:
Two holes, approximately 3 inches in diameter, were observed at the bottom of the hallway wall, adjacent to the Monkey's classroom.

Plan of Correction: Will patch and paint holes in wall.

Description: Based on observation, cleaning materials were not kept in a locked place using a safe locking method that prevents access by children.

Evidence:
In the Monkey's classroom, two cans of Lysol disinfectant spray were observed unlocked on a high shelf next to the bathrooms.

Plan of Correction: Lock away all cleaning materials.

Description: Based on review of emergency drills, evacuation drills were incomplete.

Evidence:
An evacuation drill was not practiced for the month of November 2022.

Plan of Correction: Keep up with monthly drills.
