Technical Assistance:
Technical assistance was offered related to: Staff records, sight and sound supervision; classroom transitions; and requirements for drivers.

Comments:
An unannounced non-mandated monitoring inspection was conducted from 10:10am to 12:25pm. The purpose of this inspection was to assess the correction of previously cited violations related to the care and supervision of children. There were 31 children in care of 6 staff members. The children were observed engaged in reading and circle time, playing outdoors and preparing for lunch. There were no medications on site. 3 staff records and 2 children's records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-780-340-F
Description: Based on observation, children under the age of 10 years were not always within actual sight and sound supervision of staff.
Evidence:
1. At approximately 10:40 am 4 children ran out of the 4's classroom (located at the left end of the center) as Staff # 1 was getting them lined up to go to the bathroom located across from the office and next to the kitchen.
2. The children had run out of the classroom and reached the area in front of the office and the main entrance of the building.
3. The children were stopped by Staff # 2 who was in the office. Staff # 1 joined the children with the rest of the group that she was bringing to the bathroom within less than a minute.

Plan of Correction: Extensive training on lining up and transitioning from the classroom to the bathroom and/or playground has been conducted. All staff will ensure sight and sound supervision at all times. Staff who violate this policy will be terminated.

Standard #: 8VAC20-780-350-B-4
Description: Based on observation, the staff to child ratio of 1:10 for four years old children was not maintained.
Evidence:
1. At approximately 11:00 am, the licensing inspector observed Staff # 3 from the 4's classroom and a group of children in the school age room.
2. Between 11:00 am to 11:04 am, Staff # 3 had 11 children by herself.

Plan of Correction: Staff # 3 was put on administrative leave and then terminated. Ratios must be and will be maintained at all times.

Standard #: 8VAC20-780-530-A
Description: Based on staff interview and record review, there was not at least one staff person in each area where children were present with current cardiopulmonary resuscitation (CPR) and first aid certification.
Evidence:
1. Staff # 1 first day of employment was 1/3/23.
2. Staff # 2 stated that Staff # 1 has been conducting van runs for the school age children. Staff # 2 also stated that when Staff # 1 conducts the van runs, there is no additional staff present on the van.
3. Staff # 1 did not have certification in CPR and first aid.

Plan of Correction: All staff gave received CPR and first aid. Staff # 1 was certified before returning to her van run. All van staff will receive certification before working alone with children.

Standard #: 8VAC20-780-560-G
Description: Based on observation, food and drink from home was not labeled and dated.
Evidence:
1. A bottle of raspberry lemonade was found on the shelf outside of the bathroom that was being used by children. The bottle had no name and date.
2. A bottle of a fruit smoothie that belonged to one of the children in the 4's classroom was found sitting on the shelf. The bottle did not have a name or date.

Plan of Correction: All food and drinks brought from the outside must be labeled and dated. Training was conducted and staff responsible have either been reprimanded or terminated.

Standard #: 8VAC20-780-570-B
Description: Based on observation, bottles for infants were used while the child was in their designated sleeping location.
Evidence: Child # 1 was observed drinking milk from her bottle in her crib.

Plan of Correction: Training on feeding and sleeping has been conducted. No bottles will be allowed in cribs. Staff responsible have been disciplined.

Description: Based on observation, bottles with infant formula were not dated.
Evidence: 4 infant bottles for two children with formula in the classroom were not dated.

Plan of Correction: Bottles will be dated when made. Staff have been trained to label and date all bottles.
