A non-mandated inspection was initiated on February 16, 2023 from 9:10 A.M.-11:00 P.M. There were 42 children present, ranging in ages from seven months to five years of age, with 12 staff supervising. The inspector reviewed compliance in the areas of administration, staff qualifications and training, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of one child records and four staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-160-C Description: Based on interview and review of staff records, the center failed to ensure that at least every two years from the date of the initial screening or testing, a staff member obtained and submitted the results of a follow-up tuberculosis screening.
Evidence:
1. The most recent tuberculosis test in the record for Staff #1 is dated 01/21/2021.
2. Staff #1 verified the date of the tuberculosis test on file, and that a follow-up tuberculosis screening had not been obtained.Plan of Correction: Staff #1 went that same day for the TB test and had results on 2/18/2023
Standard #: 8VAC20-780-280-B Description: Based on observation and interview, the center failed to ensure that all hazardous materials were kept in a locked location.
Evidence:
1. In Classroom #1 there was a spray bottle that was labeled bleach water sitting on the shelf above the microwave, and a can of Lysol Disinfectant Spray sitting on the second shelf next to the toilet in the restroom that the children use.
2. On the front of the can of Lysol Disinfectant Spray it states, ?Caution? and ?keep out of reach of children.?
3. Staff #3 verified that the spray bottle of bleach water and the Lysol Disinfectant spray were not in a locked location.Plan of Correction: Staff #3 removed the Lysol spray and bleach water immediately. She thought because they were higher placed the children couldn't reach them. Reminded to lock up once done using.
Standard #: 8VAC20-820-120-E-3 Description: Based on observation of posted information, the center failed ensure that the Superintendent?s intent to revoke the license of the child day center was posted in a prominent place at each public entrance of the facility to advise consumers of serious or persistent violations.
Evidence:
The Notice of Intent to Revoke License provided by the Virginia Department of Education, signed by the Director of the Office of Child Care Health & Safety on February 2, 2023, was not posted in a prominent place at each public entrance of the facility to advise consumers of serious or persistent violations.Plan of Correction: There were two NOI posted. NOI is now posted.
