Comments:
An unannounced monitoring inspection was conducted on 2/22/23 from 12:05pm until 1:55pm. At the time of the inspection, there were 81 children in care with 13 staff present. A sample of 7 children's records and 5 staff records were reviewed. Children were observed resting quietly during nap time and playing with age appropriate infant toys. Diapering, handwashing and restroom procedures were also observed. First aid and emergency supplies, the emergency preparedness plan, documentation of emergency practice drills, medication, transporation and required center postings were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law. Violations were documented on the violation notice issued to the program and discussed with the center director during the exit interview.

Violations:
Standard #: 8VAC20-780-510-E
Description: Based on a review of medication and interview, the center did not ensure that procedures for administering medication shall include methods to prevent use of outdated medication.

Evidence:
1. There is an EpiPen prescribed to a child in care being stored at the center that has an expiration date of January 2023 and liquid Benadryl that has an expiration date of October 2022.
2. The center director confirmed that the EpiPen and liquid Benadryl are expired.

Plan of Correction: The center responded with the following: The parent will be contacted to obtain new medication for the child. A medication captain has been put into place to review medication on a monthly basis to ensure expired medication is sent home and all medication policies are being followed.

Standard #: 8VAC20-780-510-I
Description: Based on a review of medication and interview, the center did not ensure that prescription medication shall be administered to a child only with written authorization from the parent.

Evidence:
1. Child 1 was administered Albuterol on 1/19/23, 1/20/23, 1/24/23 and 1/30/23 without written authorization from the parent.
2. The center director confirmed that Albuterol was administered to child 1 on the above dates without written authorization from the parent.

Plan of Correction: The center responded with the following: The parent of child 1 will be asked to sign the parent authorization form at pickup today. A medication captain has been put in place to ensure all medication policies are being followed and written authorization forms are completed and signed by the parent and physician.
