Comments:
A focused monitoring inspection was initiated on Tuesday, February 14, 2023 and concluded on Friday, February 17, 2023.There were 14 children present, ranging in ages from infancy to four years, with 2_ staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 4 child records and 3 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-780-280-B
Description: Based on observation and photograph, the center did not ensure all hazardous substance were kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. In the infant room, there was a bottle of hand sanitizer observed on the diapering area with the labels "keep out of reach" and "warning".
2. In the kitchen area, there was a bottle of odor eliminator spray with the labels "caution" and "keep out of reach of children" on the countertop accessible to children in care.

Plan of Correction: The center will ensure all hazardous substances are locked up and away from children.

Standard #: 8VAC20-780-340-F
Description: Based on observation and interview with staff, the center did not ensure children under 10 years of age are always within actual sight and sound supervision of staff.

Evidence:
1. During the inspection on February 14, 2023, Staff#3 was observed by the inspector in the kitchen upon arrival.

2. The observation of the Threes/Fours group, revealed there were 7 children in care engaged in active play activities and Staff#3 was the only staff present for the group during this inspection.

3. Interview with Staff#3 confirmed working alone with Threes/Fours group and not directly supervising the children while in the kitchen.

Plan of Correction: The center will ensure children are within proper sight and sound with adults at all times.

Standard #: 8VAC20-780-350-B-1
Description: Based on observation and interview, the center did not ensure the staff to children ratios are followed whenever children are in care.

Evidence:

1. In the Infant room on February 14, 2023, Child#1 was identified as the youngest, age 3 months and there was observation of one staff with six children.

2. Interview with staff confirmed working alone with children in care during the inspection.

Plan of Correction: The center will ensure the correct staff are in place for the children to follow the staff to children ratio for each group. The center reviewed contracts with parent to receive notice of children's attendance.

Standard #: 8VAC20-780-440-B
Description: Based on observation and interview, the center did not ensure all cribs shall be identified for use by a specific child.

Evidence: In the Infant room, there was observation of one crib without label for a specific child. Interview with staff confirmed use of the crib by a child and a label being available for review during the inspection.

Plan of Correction: The center has labeled crib for assigned child.

Standard #: 8VAC20-780-570-A
Description: Based on observation, the center did not ensure all children when placed in an infant seat or high chair protective belt is fastened securely.

Evidence: During the inspection on February 14, 2023, there was observation of two infants placed in infant seat without the protective belt fastened.

Plan of Correction: The center review safety procedures with staff to ensure protective strapes are secured and to follow center procedures.
