Comments:
A monitoring inspection was conducted on 02/21/2023. There were 43 children present, ranging in ages from infant to 5 years, with 6 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 4 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the owner who provided the plans of correction during the exit interview.

Violations:
Standard #: 8VAC20-780-280-B
Description: Based upon observation and staff interview. the facility has not ensured that hazardous substances are kept in locked locations using safe locking methods that prevent access by children.
Evidence:
1. There was a spray bottle of air freshener and another container of spray disinfectant, both with warning labels, in the unlocked staff bathroom in the girls' bathroom.
2. Staff 2 verified the unlocked substances in the girls' bathroom
3. There was a container of spray disinfectant with a warning label on a high shelf in the unlocked bathroom adjoining the toddler room.
4. Staff 1 verified the presence of the disinfectant.

Plan of Correction: The facility responded with the following:
All potentially hazardous substances were placed in locked locations during the inspection. All staff will be re-trained regarding identifying and acceptable storage (locked) of potentially hazardous substances

Standard #: 8VAC20-780-510-E
Description: Based upon review of medication, the facility has not ensured that their medication policies are followed.
Evidence:
1. The parent authorization for emergency administration of an asthma inhaler to child 2 did not include the duration of the parent's authorization.
2. The allergy action plan for child 6 indicated to administer diphenhydramine, followed by an EpiPen should the diphenhydramine not be effective. There was no diphenhydramine for child 6.
3. There was no written uthorization for an EpiPen to be administered in an emergency to child 6.

Plan of Correction: The facility responded with the following:
When bringing in medication to be administered to a child, parents will be required to complete a separate parent authorization form to ensure that there is proper documentation. Allergy action plans will be reviewed with the parent to ensure that required actions can be followed.

Standard #: 8VAC20-780-570-E
Description: Based upon observation, the facility has not ensured that prepared bottles are labeled with the date.
Evidence:
There was a prepared bottle in the infant room for child 4 that was dated 2/13.

Plan of Correction: The facility responded with the following:
All infant staff will be reminded that prepared formula must be labeled with the child's name and the correct date. If a bottle is brought in from another classroom, infant staff must ensure accurate labeling of the bottle.
