Technical Assistance:
Technical assistance was given and Licensing Standards were reviewed regarding cords to blinds for the window.

Comments:
An unannounced renewal inspection was conducted on February 21, 2023, from 10am to 1pm. There were 33 children and 6 staff members present. The Inspector reviewed five children?s records and five staff records. The Inspector observed staff and children?s interactions throughout the inspection. Children were observed during group activity, lunch, free play, diapering, and naptime. There is no medication being administered at the center at this time. There is no transportation provided at this time. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented. The violations are listed on the violation notice issued to the center and were reviewed with the owner at the exit interview.

Violations:
Standard #: 8VAC20-780-350-B-1
Description: Based on observation and interview, it was determined that the center did not ensure that the staff-to-children ratio and group size requirements for infants birth up to 16 months is at the ratio of 1:4 (staff:children).
Evidence:
1. In the infant room, there were 10 infants observed and two staff members in which the ratio is to be 1 staff member to every 4 children in care.
2. Infant staff members confirmed that they were out of ratio.
3. Staff #1, the center director, confirmed that the infant room was out of ratio with 10 children to two staff members when the required ratio is one staff member to every 4 children in care.

Plan of Correction: The center responded with the following: Staff #1 stated that the additional staff for the infant room was present however had to assist with another classroom as another staff member, a preschool teacher, did not show up to work. The preschool teacher was having car issues and it was understood that they were on the way to work but never showed up. Staff #1 will ensure that if a staff member is running late, the ratio will be accounted for prior to accepting additional children into the center. The parents will be asked to wait until a staff member shows up or not be able to accept them into care due to staffing shortages. Staff #1 has a toddler teacher and a prek teacher currently in the hiring process. Staff #1 will also look into a volunteer and/or floater to ensure that ratios are always per licensing standards.

Standard #: 8VAC20-780-350-B-4
Description: Based on observation and interview, it was determined that the center did not ensure that the staff-to-children ratio and group size requirements for children ages 3 years old up to school age is at the ratio of 1:10 (staff:children).
Evidence:
1. In the preschool classroom, there were 13 children to 1 staff member observed in which the required ratio is 10 children to 1 staff member. This room was out of ratio.
2. Staff #1 confirmed that the preschool classroom was out of ratio and that there were 13 children to 1 staff member.

Plan of Correction: The center responded with the following: Staff #1 stated that the additional staff for the infant room was present however had to assist with another classroom as another staff member, a preschool teacher, did not show up to work. Description: Based on record review and interview, it was determined that the center did not ensure that they implement a monthly practice evacuation drill.
Evidence:
1. There was no practice evacuation drill documented for January 2023.
2. Staff #1 confirmed that there was no practice evacuation drill conducted for January 2023.

Plan of Correction: The center responded with the following: The center will ensure that all practice evacuation drills be implemented monthly and timely.

Standard #: 8VAC20-780-560-F
Description: Based on observation and interview, it was determined that the center did not ensure that a menu listing foods to be served for snacks during the current one-week period be posted in a location conspicuous to parents or given to parents.
Evidence:
1. In walking throughout the center, there was no posting of the snack menu for the current one-week period in a location conspicuous to parents or given to parents.
2. Staff #1 and Staff #2 confirmed that there was no menu posted in the center.

Plan of Correction: The center responded with the following: Moving forward, the center will post the snack menu on a week by week basis.
