Standard #: 22.1-289.035-A Description: Based on review of ten staff records, the program did not ensure that all employees complete background checks every five years for one staff.
Evidence:
1. The first day of employment for Staff #7 was 12/14/2014. The first central registry check in the record was dated 12/12/2014. The current central registry check in the record was dated 2/14/2020.Plan of Correction: Ongoing Corrective Action Steps:
The current center administration has a spreadsheet tracking system in place to ensure that central registry checks are not outdated.
Standard #: 22.1-289.035-B-2 Description: Based on review of ten staff records, the program did not ensure that a criminal record check was completed prior to first day of employment for one staff.
Evidence:
1. The first day of employment for Staff #3 was 5/31/2022. The criminal record check in the file was dated 6/1/2022.Plan of Correction: Ongoing Corrective Action Steps:
Center administration have been retrained and are being reviewed on how to
complete the background checks. During this retraining and review period, the
center administration will submit all new employee?s background checks for
review with the regional manager to check for accuracy and compliance before
the date of hire
Standard #: 22.1-289.035-B-4 Description: Based on review of ten staff records, the program failed to ensure that a criminal history record check from any state of residence in the preceding five years is completed prior to employment for one staff.
Evidence:
1. The first day of employment for Staff # 10 was 1/23/2023. There was no out of state criminal history check in the file.
2. The administrator confirmed it had not been requested, and LI assisted in finding the contact information to request.Plan of Correction: Program?s Immediate Response:
Notice given to regional manager/supervisor and the following actions were taken immediately:
Worked with licensing inspector?s assistance to find out-of-state contact information for staff #10. Record check was submitted immediately and received back on 2/27/2023.
Standard #: 8VAC20-780-130-A Description: Based on the review of ten child records, the program failed to ensure that immunization records where on file prior to first day of attendance for one child.
Evidence:
1. The first day of attendance for Child #7 was 11/6/2022. The immunization record in the file was dated 1/5/2023.Plan of Correction: Ongoing Corrective Action Steps:
New children enrolling at the center will not be able to attend until a complete
physical, including immunizations, are submitted for review. A spreadsheet
tracking system will monitor annual physicals and upcoming immunizations due.
