A monitoring inspection was conducted on March 2, 2023. There were 64 children present, with 14 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 4 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 22.1-289.035-B-4 Description: Based on review of staff records, the child day center did not obtain a copy of the results of a search of the child abuse and neglect registry for any state in which the individual has resided in the preceding five years.
Evidence
Staff #1 (DOH 11/21), has lived out of the state of the Virginia in the preceding five years does not have documentation of a search of the child abuse and neglect registry in the state they had lived.Plan of Correction: We will follow up on the missing out of state background check. It was submitted, but not returned. I will let the inspector know when it is received.

Standard #: 8VAC20-780-280-B Description: Based on observation, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children.

Hazardous substances, adhesive spray, was accessible to children in the Toddler 2 classroom. Spray was located in an unlocked drawer by the sink area.Plan of Correction: We will speak with staff to ensure all cabinets are checked and relock when done.
Standard #: 8VAC20-780-420-E-1 Description: Based on review of infant daily records, the center is not recording for infants, who are awake and cannot turn over by themselves, the amount of time spent on their stomachs.

For any infant who can't turn over by themselves, there is not recording of time they spend on their stomachs.Plan of Correction: We will speak to staff to ensure all tummy time is being recorded.
Standard #: 8VAC20-780-520-A Description: Based on observation, over-the-counter skin products were kept beyond the expiration date of the product.

Expired over-the-counter skin products were located in the Toddler 1 and Toddle 2 classrooms.Plan of Correction: We will remove all expired topicals. We will check all topicals on a regular schedule to make sure they are not expired.
