Comments:
An unannounced renewal inspection was conducted on March 2, 2023. There were 33 children present with 9 staff supervising. The physical plant, children's records, staff records, emergency drill logs, attendance logs, allergy list, menu's, evacuation preparedness procedures, bottles, cribs and medication and authorizations were reviewed. The children were observed working on lessons, listening stories and playing with floor toys. Areas of non-compliance were cited on the violation notice. Violations:
Standard #: 8VAC20-780-60-A
Description: Based on review of children's records, the center failed to ensure that all required documentation is obtained for each child enrolled.

Evidence:
Child #3 and #5's records did not contain documentation of the work phone number of each parent who has custody.

Plan of Correction: Work numbers were added during the inspection.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center failed to ensure that areas and equipment of the center inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.

Evidence:
1. In the Jr. Toddler room, a blue chair has cracked on the front.
2. In the Jr. Toddler room, the changing table is soiled.

Plan of Correction: The chair will be discarded today.

The changing table will be cleaned today.

Standard #: 8VAC20-780-290-A-3
Description: Based on observation, the center failed to ensure that electrical outlets shall have protective covers that are of a size that cannot be swallowed by children.

Evidence:
In the Infant room, in the nap area, there is an outlet cover missing.

Plan of Correction: Outlet cover replaced during inspection.

Description: Based on observation, the center failed to ensure that there is a nonabsorbent surface for diapering or changing shall be used.

Evidence:
In the Jr. Toddler classroom, the changing pad had a hole in it and therefore is no longer nonabsorbent.

Plan of Correction: Changing pad will be replaced.

Standard #: 8VAC20-780-560-G
Description: Based on observation, the center failed to ensure that when food is brought from home, the food container shall be sealed and clearly dated and labeled in a way that identifies the owner.

Evidence:
In the Infant room, there were bottles and food containers that had children's name, but were not dated.

Plan of Correction: The bottles and food containers were dated during the inspection.
