Comments:
An unannounced monitoring visit and facility tour was conducted on 2/15/2023. At the time of inspectors' arrival there were 18 school aged children in care with 2 staff members. Children were observed interacting with staff, playing outdoors, and departing. A sample of 5 children's records and 3 staff records were reviewed. Director reported that there have been no injuries and medications are not being administered at this time. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Violations:
Standard #: 8VAC20-770-60-C-2
Description: Based on record review and interview it was determined the center did not ensure each staff member does not have a central registry finding within 30 days of employment or volunteer service.
Evidence:
1. The staff record for staff #1(hired 8/18/22) did not contain documentation of a central registry search.
2. Staff confirmed these documents were not present during inspection.

Plan of Correction: Director will follow up on background check.

Standard #: 8VAC20-780-60-A
Description: Based on record review it was determined the center did not ensure that a separate record for each child enrolled contained all of the required information.
Evidence:
1. The records for child #1, child #2, and child #3 did not contain the addresses for the two required emergency contacts.
2. The records for child #1 and child #2 did not contain proof of identity.
3. There record for child #5 did not contain:
a. Home address and home phone number of each parent;
b. Work phone number and place of employment of each parent;
c. Previous child day care and schools attended.
d. Name, address and phone number of two designated people to call in an emergency;
e. Names of persons authorized to pick up the child;

Plan of Correction: Director will request documentation from parents.

Standard #: 8VAC20-780-90--A
Description: Based on record review and interview it was determined the center did not ensure each child's record contained signed written agreements between the parent and the center by the child's first day of attendance as required.
Evidence:
1. The record for child #5 did not contain documentation of the required written agreements.
2. Staff confirmed this information was not available during this inspection.

Plan of Correction: Director will request documentation from parent.
