Technical Assistance:
Discussed clear documentation required for head trauma procedure as it relates to 8VAV20-780-40.M.

Comments:
A monitoring inspection was initiated on 3-1-2023 and concluded on 3-1-2023. There were 35 children present with 7 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program

Violations:
Standard #: 8VAC20-780-500-A
Description: Based on observation and staff interviews, the licensee did not ensure children?s hands shall be washed with soap and running water after toileting and any contact with blood, feces, or urine.
Evidence: Staff #4 was observed conducting a diaper change for Child #6 and the child?s hands were not washed after diaper change. The child was observed touching the soiled diaper.

Plan of Correction: Again staff can get nervous while being observed. I am posting a new chart with steps so if they get overwhelmed they have steps in front of them.

Standard #: 8VAC20-780-500-B
Description: Based on observation and staff interview, the licensee did not ensure the diapering surface shall be cleaned with soap and at least room temperature water and sanitized after each use.
Evidence: Staff was observed, after a diaper change, spraying a bleach and water mixture on the diapering surface and immediately wiping it off. Staff confirmed they did not clean the surface first with soap and water and did not allow the sanitizer to stay on the surface for 2 minutes before wiping it off.

Plan of Correction: Discussed with staff. We will now have bottle #1 marked soap and water and bottle #2 marked bleach and water. When being observed and getting nervous a reminder is in front of them. Discussed drying time.
