Comments:
A mandated monitoring inspection was conducted on February 28, 2023 from 3:15 P.M.-5:50 P.M. There were 50 children present, ranging in ages from five to twelve years of age, with nine staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and seven staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-40-J
Description: Based on interview and review of the injury prevention plan, the center failed to update the injury prevention plan at least annually.

Evidence:
1. The Injury Prevention on file was last dated August 28, 2021.
2. Staff #1 verified that he injury prevention had not been updated in 2022.

Plan of Correction: The injury prevention plan has been updated and will be distributed to all staff. In the future the injury prevention plan will be reviewed and updated at least annually.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center failed to maintain all areas in a safe and operable condition.

Evidence:
1. In the Green Classroom there was a white and blue shelve in that was not stable or sturdy. The shelve was leaning to the right and would not stand straight.
2. In the upstairs girls restroom, in the bathroom stalls there were two large holes in both of the stalls right below the toilet paper holder. The stall on the right had a large patch of peeling paint.
3. Upstairs in the foyer area between the restrooms and the Purple Classroom there were several areas on the wall that had peeling paint.

Plan of Correction: Administration will contact the church and put a plan in place to have the areas repainted. The shelve was removed from the classroom.
