An unannounced monitoring was conducted on March 1, 2023 from approximately 10:47am -11:33 am. There were 12 children present upon arrival with 2 staff members. Children were observed playing outside, washing hands and eating lunch. Two staff records and three children's records were reviewed. Licensing Inspector reviewed indoor and outdoor areas, supervision, activities, equipment, medication procedures, emergency evacuation and shelter-in-place drill documentation, emergency procedures and required posting.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 8VAC20-780-160-A Description: Based on record review, the licensee did not ensure each staff member and individual from an shall submit documentation of a negative tuberculosis screening be submitted at the time of employment and prior to coming into contact with children and documentation needs to be completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.
Evidence:

Standard #: 8VAC20-780-280-B Description: Based on observation , the licensee did not ensure hazordous substances were kept in a locked place using a safe locking method that prevents acces to children.

Standard #: 8VAC20-780-550-G Description: Based on review of documentations, the licensee did not ensure there was documentation of emergency evacuation , shelter in place and lcokdown drills.
Evidence:
The center documentation did not show a monthly emergency evacuation drills were completed and once a year lockdown drill.
Confirmed by staff #1 they conducted evacuation drills with the school .
