Comments:
An unannounced renewal inspection was conducted on 03/02/2023 between the hours of 11:20am-3:15pm. There were 54 children directly supervised by 9 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, and emergency supplies were inspected. Children were observed participating in free play, reading books, completing puzzles, and preparing for lunch. Hand-washing procedures were also observed. There was an assortment of age appropriate materials for the children in care. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-780-240-C
Description: Based on record review, orientation training did not include required facility specific topics.

Evidence:
Staff #1 began employment on 2/13/2023. There was no documentation of orientation training addressing the required topics as they pertain to this facility, in the record for Staff #1.
Documentation of an orientation training from a previous location was included in the record but doesn?t address the required facility specific components.

Plan of Correction: The orientation training will be completed tomorrow.

Standard #: 8VAC20-780-270-A
Description: Based on observation, areas inside the center were not maintained in a safe condition.

Evidence: Chipped and peeling paint was observed on the wall of the Infant 1 room.

Plan of Correction: A maintenance request to paint the wall will be submitted today.

Standard #: 8VAC20-780-510-L
Description: Based on observation, medication was not kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. Medication stored in the preschool room was located in an unlocked emergency bag in the classroom.
2. The lock on the bag in which the medication was stored was broken and inspector was able to gain access to the medication.

Plan of Correction: A replacement container with a functioning lock was obtained during the inspection and the medication was placed in the locked container.
