Comments:
An unannounced on-site renewal inspection was conducted on 03/07/2023. There were 40 children, ages 6 months-6 years, and 7 staff members present during the inspection. The inspector observed the following: free choice activities, large motor play outside, diapering, lunch and napping. A total of 5 children?s records and 5 staff records, were reviewed.The inspector discussed the following with staff: staff records, storage of staff items, and playground safety. Due to an allowable variance being denied on November 4, 2022 for use of floor beds for infants and a repeat violation of 8VAC20-780-(6)-440-G, an Intensive Plan of Correction (IPOC) will be required.
The inspector arrived at 11 AM and departed at 2:20 PM.

Violations:
Standard #: 8VAC20-780-40-N
Description: Based on review of the playground safety plan, the center failed to ensure that all required components were part of the plan.

Evidence:
The playground safety plan did not contain the method of maintaining resilient surfacing.

Plan of Correction: The director added the method of maintaining resilient surfacing to the plan during the inspection.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the center failed to ensure that all chemicals were not locked using a safe locking method that prevents access by children.

1.In the main office of the center there was a gallon container of a cleaning chemicals (Lysol) sitting on the floor as you enter the office. The door through out the day is open and not locked.
2.In the infant 1 room there was two spray containers of cleaning chemicals (soap and water and sanitizer) sitting on the diaper changing table. The infants were finishing up lunch and infants were not being diapered.

Plan of Correction: The director will inform all staff that all cleaning supplies are to remain locked unless they are diapering. All chemicals will be stored in the locked cabinet.
The office door is locked when an adult is not in there.

Standard #: 8VAC20-780-440-G
Description: REPEAT VIOLATIOIN

Based on observation and discussion with the staff, the center failed to ensure that cribs shall be used for children under 12 months of age.

Evidence:

1. On the day of the inspection, there were three children under 12 months sleeping on a floor bed and not in the required crib per the standard. Child # 3 was 6 months old, child # 4 was 7 months old and child # 5 was 10 months old.
2. Staff # 4 confirmed that cribs have never been used for infants.

Plan of Correction: An IPOC (Intensive Plan OF Correction) will be required.

Standard #: 8VAC20-780-500-B
Description: Based on observation, the center failed to ensure that all diapering standards were met.

Evidence:

Observed a child in the young toddler room having their diaper changed. Staff # 2 was sitting on the floor in between the door and the lunch tables. The child was doing a stand-up diaper change. There was no mat on the ground during this change and the child was approximately 3-4 inches away from children eating lunch.

Plan of Correction: Both of our toddler rooms are the same, we do not have a ?young?
toddler room. The student was in underwear and had an accident.
We will advise staff that it is best to remove students from the area to
avoid contaminants, as we are not able to control where accidents
occur.

Standard #: 8VAC20-780-560-G
Description: Based on observations, the center failed to ensure that all items brought from home were labeled and dated.

Evidence:

In the infant room refrigerator, there were 3 bottles observed. One bottle was not labeled or dated. Staff # 4 stated it was breast milk that belonged to another staff person.

Plan of Correction: Items will not go into the fridge unless labeled. All staff was immediately reminded.
