An unannounced self-reported inspection was conducted on March 7, 2023, regarding concerns of care and protection and supervision of a child. Staff interviews were conducted and video footage was reviewed. Based on the evidence gathered, violations were cited, along with other violations observed during the inspection. Standard #: 22.1-289.035-B-4 Description: Based on review of staff records, the center did not obtain a copy of the results of a criminal history record information check and a search of the child abuse and neglect registry from any state in which the individual has resided in the preceding five years.
Evidence
The center did not obtain out of state results of a criminal history record information check and a search of the child abuse and neglect registry for staff #3, who has lived in two other states in the preceding five years.Plan of Correction: Although steps have been taken previously to obtain the missing background checks, the process will be initiated again and follow up will be made if not received within 30 days.
Standard #: 8VAC20-780-70 Description: Based on review of staff records, two did not contain the required documentation.
Evidence
Staff #1 and staff #2 do not have documentation of two references being checked. before first day of employment. Center confirmed only one reference was checked for staff #1 and #2.Plan of Correction: Reference checks will be completed for the noted staff by March 13, 2023.
For all new staff, reference checks will be required prior to their first day of employment.
Standard #: 8VAC20-780-340-A Description: Based on review of electronic video footage, when staff were supervising children, they did not always ensure their care, protection and guidance.
Evidence
Staff #1 was the only staff in the classroom and was supervising child #1,who was the only child in the classroom.
Staff #2 entered the classroom and placed something on the counter and walked out without seeing child #1.
Staff #3 opened the door to the classroom to inform staff #1 that she was needed in another area. Staff #1 walked out of the classroom, leaving child #1 in the room.
Staff #3 also walked out of the classroom without seeing child #1.
.Plan of Correction: All staff are being retrained on proper supervision of children to ensure their care, protection and guidance. All staff will be taking the courses Quality Supervision and Supervision: Staff/Child Ratios provided by Better Kid Care. All staff are required to complete these courses as well as the new Virginia Annual Health and Safety Update Training for Child Care Staff (2023) by March 31st.
Procedurally, staff is being retrained to communicate with each of upon entering or exiting a classroom to ensure that all staff in that room are aware of their presence or absence and that there is a proper handoff for the care of the children in that room and that ratios are being maintained.
