An unannounced on-site monitoring inspection was conducted on 03/09/2023. There were 108 children, ages 6 months-6 years, and 22 staff members present during the inspection. The inspector observed the following: free choice activities, large motor play outside, diapering, and lunch. A total of 5 children?s records and 7 staff records, were reviewed. The inspector discussed the following with staff: immunization records and required signatures, Allowable Variance and DHO training. The inspector arrived at 9:40 am and departed at 12:15 pm

Standard #: 8VAC20-780-280-B Description: Based on observation, the center failed to ensure that all chemicals were locked using a
safe locking method that prevents access by children.
Evidence:
1.Under the sink in the Cedar room there were 12 chemicals that were unlocked. The following were unlocked: Bee Garden Spray (insecticide), Garden Dust (insecticide), Neem Oil (insecticide), Cintas disinfectant & cleaner, Clorox wipes, Stain and Odor Remover Simple South, Fabuloso, Cascade, Lemon Fresh Disinfectant, Signet Hard Surface Sanitizer, Multi Surface glass cleaner and a pink unlabeled container with a chemical.
2.Under the sink in the Sequoia room there were 8 chemicals that were unlocked. The following were unlocked, Clorox wipes, Odor Ban, 2 containers of Mircoban (an aerosol bottle and a spray bottle) , Cascade, Signet Hard Surface Sanitizer, Windex glass cleaner and a clear glass unlabeled container with a chemical.
3. A cabinet above the counter in the Blue Spruce room there were 8 chemicals that were unlocked. The following were unlocked and accessible to children: Clorox wipes, Cascade, Two containers of Signet Hard Surface Sanitizer, Odor Ban, Lysol aerosol, glass cleaner and a soap and water spray bottle.Plan of Correction: The director will at the weekly and monthly staff meetings remind staff of checking behind yourself when locking the cabinets. Director made sure during the inspection that all unlocked chemicals were locked. Director and staff will do mock inspections to monitor all chemicals.
Standard #: 8VAC20-780-280-E Description: Based on observation, the center failed to ensure that cleaning materials and insecticides/ pesticides were stored in areas physically separate from each other.
Evidence:
1.Under the sink in the Cedar room there were 12 chemicals that were unlocked and stored together. There were three insecticides/pesticides stored with 9 cleaning chemicals. The following were stored together: Bee Garden Spray (insecticide), Garden Dust (insecticide), Neem Oil (insecticide), Cintas disinfectant & cleaner, Clorox wipes, Stain and Odor Remover Simple South, Fabuloso, Cascade, Lemon Fresh Disinfectant, Signet Hard Surface Sanitizer, Multi Surface glass cleaner and a pink unlabeled container with a chemical.Plan of Correction: The director will at the weekly and monthly staff meetings remind staff of the correct place to store insecticides. Director made sure during the inspection all insecticides were removed and stored in the shed. Director and staff will do mock inspections to monitor all chemicals
Standard #: 8VAC20-780-280-G Description: Based on observation, the center failed to ensure that if hazardous substances are not kept in the original containers that the substitute containers shall clearly indicate their contents.
Evidence:
1.In the Cedar room there was clear liquid in a pink spray bottle. The LI smelled the clear liquid and it appeared to be bleach water.
2.In the Aspen room there was clear liquid in a clear glass spray bottle. The LI smelled the clear liquid and it appeared to be Pine sol.Plan of Correction: The director will at the weekly and monthly staff meetings remind staff of labeling all substitute containers clearly. Directors will make sure that today all substitute containers are labeled with their contents. Director and staff will do mock inspections to monitor all chemicals
Standard #: 8VAC20-780-500-A Description: REPEAT VIOLATION
Based on observation, the center failed to ensure that children?s hands were washed with soap and running water or use disposable wipes before and after eating meals and snacks.
Evidence:
1.Observed in the Aspen room five children started eating lunch and the children?s hands were not washed prior to eating with soap and running water or a disposable wipe.
2.Observed in the Evergreen room ten children were started eating lunch and the children?s hands were not washed with soap and running water or a disposable wipe.Plan of Correction: The director will at the weekly and monthly staff meetings remind staff of the procedure of sanitizing tables. Director and staff will do mock inspections to monitor all with standards.
Standard #: 8VAC20-780-500-B Description: REPEAT VIOLATION
Based on observation, the center failed to ensure that all diapering standards were met.
Evidence:
1.Observed a staff # 5 discard a soiled diaper in the trash can by using their hand to open the lid.
2.There were children from 6 months to 3 years present at the center on day of the inspection and there was no changing table or countertop designated for changing. Observed diapering on the floor with a changing pad and stand-up diapering.Plan of Correction: The director will at the weekly and monthly staff meetings remind staff of the procedure of diapering procedures. Director and staff will do mock inspections to monitor all with standards.
An AV will be completed or diaper changing table or counter will be purchased.
Standard #: 8VAC20-780-560-J Description: Based on observation, the center failed to ensure that all tables are sanitized before and after each use for feeding.
Evidence:
Observed in the Aspen room and the Evergreen room, the tables were not sanitized prior to eating lunch.Plan of Correction: The director will at the weekly and monthly staff meetings remind staff of the procedure of sanitizing tables. Director and staff will do mock inspections to monitor all with standards.
