Comments:

An unannounced monitoring inspection was conducted today between the hours of 9:45am-11:45am. There were a total of 97 children with 15 supervising staff members present. Children were observed during diapering, handwashing, toys, engaged in group activities and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Standard #:	8VAC20-770-60-C-2
Description:	Based on review of three staff records and interview, the center did not have an original Central Registry Finding within 30 days of employment of each staff person. Evidence: 1. Staff #1 (Date of hire-09/22/2022) did not have a Central Registry finding within 30 days of employment. A Central Registry finding was dated 11/11/2022. There was no documentation of a follow up after 30 days. 2. Staff #2 (Date of hire-09/07/2022) did not have a Central Registry finding within 30 days of employment. A Central Registry finding was dated 10/26/2022. There was not documentation of a 30 day follow up. 3. Staff #3 (Date of hire-09/09/2022) did not have a Central Registry finding within 30 days of employment. A Central Registry finding was dated 10/27/2022. There was no documentation of a 30 day follow up. 4. Staff #6 confirmed that the Central Registry findings were not obtained within 30 days of employment and that a 30 days follow up with the Office of Background Investigations was not conducted or documented.
Plan of Correction:	Administration will ensure to follow up with the Office of Background of Investigations after 30 days of a Central Registry Search if not returned.

Description:	Based on record review, the center did not obtain documentation of additional immunizations once every six moths for children under the age of two years. Evidence: 1. Child #2's record (Start date-07/18/2022) did not contain documentation of additional immunizations. The last immunization record was dated 07/14/2022.
Plan of Correction:	The parents will be contacted to get additional immunizations.

Description:	Based on record review, required records were not kept for each staff person. Evidence: 1. Staff #5's record (Date of hire-03/03/2023) did not contain documentation of a second reference check before employment.
Plan of Correction:	A second reference will be obtained for the staff member.

Description:	Based on record review, the program leader does not meet the qualifications of a program leader. Evidence: 1. Staff #3 (Date of hire-09/09/2022) is listed as a program leader for Classroom #4 (Fantastic Frogs). Staff #3 does not have documentation in the record that qualifies them as a program leader.
Plan of Correction:	Administration will work on getting documentation to qualify the lead teacher as a program lead.
Standard #:	8VAC20-780-240-A
Description:	Based on record review, staff did not complete the Virginia Department of Education sponsored orientation course within 90 calendar days of employment. Evidence: 1. Staff #1 (Date of hire-09/22/2022) did not complete the Virginia Department of Education sponsored orientation course within 90 days of employment. Staff #1 completed the orientation course on 01/23/2023. 2. Staff #2 (Date of hire-09/07/2022) did not complete the Virginia Department of Education sponsored orientation course within 90 days of employment. Staff #2 completed the orientation course on 01/20/2023. 3. Staff # 3 (Date of hire-09/09/2022) did not complete the Virginia Department of Education sponsored orientation course within 90 days of employment. There was not documentation that the course had been completed in the record.
Plan of Correction:	Administration will ensure that new staff members will complete the VDOE sponsored orientation course within 90 days. The staff member will complete the VDOE sponsored orientation course.
Standard #:	8VAC20-780-270-A
Description:	Based on observation, areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Evidence: 1. In Classroom #6(Outstanding Owls), a rug located in the home living area is unraveling on the two right corners causing a tripping hazard.
Plan of Correction:	The rug was removed form the classroom during the time of the inspection.
Standard #:	8VAC20-780-510-P
Description:	Based on review of medications, authorization for medication was expired. Evidence: 1. Child #6 has emergency medication on site. The authorization expired on 10/05/2022.
Plan of Correction:	The medication will be picked up by the parents and a new authorization will be obtained.
Standard #:	8VAC20-780-520-C
Description:	Based on observation, diaper cream was accessible to children. Evidence: 1. In Classroom #3 (Precious Pandas), a container of diaper cream was located in the bathroom in a cabinet with a broken door. The diaper cream was located on a low shelf of the cabinet making it accessible to children.
Plan of Correction:	The container of diaper cream was removed from the bathroom and placed in a location inaccessible to children.
