Comments:
An unannounced non-mandated inspection, required by the Conditional License, was conducted on 03/08/2023. There were 41 children, ages 4 months-5 years, and 10 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies and nutrition. The children were observed during lunch and rest time; infants were observed in play and sleeping and eating on demand. A total of 5 children?s records and 5 staff records were reviewed. The inspector arrived for the inspection at 11:20 am and departed at 2:15 pm.

Information gathered during the inspection determined non-compliance with applicable standards or laws and violations were documented on the violation notice issued to the provider.

Violations:
Standard #: 8VAC20-780-130-E
Description: Based on document review, the center failed to obtain documentation of additional immunizations once every six months for children under the age of two years.

Evidence: Child 5 was confirmed to be 13 months old. The most recent documentation of immunizations in the record for Child 5 was dated 07/18/2022. The documented first date of attendance for Child 5 was 07/25/2022. Documentation of additional immunizations were not obtained every six months for a child under two years.

Plan of Correction: Will create a spreadsheet to track.

Standard #: 8VAC20-780-150-B
Description: Based on document review, the center failed to ensure that each report of physical examination and immunizations shall include the date of the physical examination and the date the immunizations were received and shall be signed by a physician, his designee or an official of a local health department.

Evidence:
1. The immunization record in the record for Child 1 was not signed by a physician, his designee or an official of a local health department.
2. The immunization record in the record for Child 4 was not signed by a physician, his designee or an official of a local health department.

Plan of Correction: Will add the information about the requirement to the enrollment packet to get the correct information.

Standard #: 8VAC20-780-60-A
Description: Based on record review, the center failed to ensure that the separate record for each enrolled child contains all of the elements as required by the standards.

Evidence:
1. The record for Child 3 did not contain the name, address and phone number of two designated people to call in an emergency if a parent cannot be reached (the addresses of both designated people were incomplete).
2. The record for Child 4 did not contain information regarding allergies and intolerances to medication or any other substances and actions to take in an emergency.

Plan of Correction: All records will be reviewed and updated.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the center failed to ensure that hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence: There was a clear plastic spray bottled labeled ?Clorox and water? on the diaper changing table of the Toddler Room. The bottle was accessible to the children. Children were observed in the diaper changing area of the classroom.

Plan of Correction: Will retrain the staff.

Standard #: 8VAC20-780-340-D
Description: Based on record review and staff interviews, the center failed to ensure that in each grouping of children at least one staff member who meets the qualification of a program leader or program director shall be regularly present.

Evidence: The Toddler Class was observed in the care of Staff 2 and Staff 5. Staff 6 that Staff 2 was the program lead and that Staff 5 was a program aid. The review of the record for Staff 2 determined that Staff 2 did not meet the qualifications of a program leader or program director. The review of the record for Staff 5 determined that Staff 2 did not meet the qualifications of a program leader or program director. There was not at least one staff member who meets the qualification of a program leader or program director regularly present with the Toddler Class.

Plan of Correction: Will have a program lead in the class.

Standard #: 8VAC20-780-500-A
Description: Based on observations, the center failed to ensure that children?s hands shall be washed with soap and running water or a disposable wipe before and after eating meals or snacks.

Evidence: The Toddler Class was observed finishing lunch, immediately returning to their classroom, and getting on their cots for rest time. The children in the Toddler Class did not wash their hands with soap and running water or a disposable wipe after eating lunch.

Plan of Correction: Will have the staff participate in training.

Standard #: 8VAC20-780-530-A
Description: Based on record review, the center failed to ensure that at least one staff in each classroom or area where children are present shall have current certification in CPR and first aid.

Evidence:
1. There was not at least one staff member with a current certification in CPR and First Aid in the Toddler Class.
2. There was not a least one staff member with a current certification in CPR and First Aid in the 2-3 year old class.

Plan of Correction: Will have everyone get updated certification.

Standard #: 8VAC20-780-560-F-4
Description: Based on observation, the center failed to ensure that children three years of age or younger may not be offered foods that are considered to be potential choking hazards.

Evidence: The Toddler Class and the 2 Year Old Class was observed during lunch. The children, ages 16 months-2 years, were served fruit cocktail that contained whole grapes; whole grapes are considered to be potential choking hazards.

Plan of Correction: Will cut up the grapes.
