A non-mandated inspection, resulting from a self-reported incident, was conducted on 03/08/2023. A total of one staff was interviewed and one staff record was reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard #: 22.1-289.035-B-2 Description: Based on document review and staff interviews, the center failed to ensure that no person shall be hired for compensated employment prior to the facility obtaining an employment eligibility letter upon the completion of a fingerprint background check as required by the Code of Virginia.
Evidence: The record for Staff 2 contained a documented hire date of 02/17/2023. There was not an employment eligibility letter available for review during the 03/08/2023 inspection. Staff 1 stated that Staff 2 had been an employee that had worked at the center but was no longer an employee. Staff 1 stated that Staff 2 had never completed a fingerprint background check.Plan of Correction: Will get fingerprint prior to hire.
Standard #: 8VAC20-780-160-A Description: Based on record review, the center failed to ensure that each staff member shall submit documentation of a negative tuberculosis screening prior to coming into contact with children.
Evidence: The record for Staff 2 contained a documented hire date of 02/17/2023. There was not documentation of a negative tuberculosis screening available for review during the 03/08/2023 inspection. Staff 1 stated that Staff 2 had worked at the center but was no longer an employee.Plan of Correction: Will get prior to hire.
Standard #: 8VAC20-780-70 Description: Based on record review, the center failed to ensure that all staff records contain all of the elements as required by the standards.
Evidence: The record for Staff 2 did not contain the following information: documentation that two or more references as to character and reputation as well as competency were checked before employment; documentation to demonstrate that the individual possesses orientation training; and date of separation from employment.Plan of Correction: The staff is no longer employed. Will get all information prior to hire.
