Standard #: 8VAC20-770-40-D-3-d Description: Based on review, 1 of 7 staff files reviewed indicated that it has been more than five years since the date of the most recent central registry finding (CPS check) on file at the Child Day Center (CDC).
Evidence -
1. On the date of inspection (03/08/2023), the most recent CPS check on file for Staff #6 was dated - 09/19/2017.
2. The Center Director stated that action has already been taken to obtain a current finding.Plan of Correction: A request has been sent. Once the finding as been received, it will be placed on file.
Standard #: 8VAC20-780-160-C Description: Based on review, 1 of 7 staff files indicated that it has been more than 2 years since the date of the current TB documentation on file at the center.
Evidence -
1. On the date of inspection (03/08/2023), the current TB documentation on file for Staff #4 was dated - 11/25/2019.
2. This was confirmed on the date of inspection by the Center Director.Plan of Correction: Updated TB documentation will be obtained.
Standard #: 8VAC20-780-60-A Description: Based on review, 1 of 4 children files indicated that the health forms (immunization page and documentation of a physical examination) was not contained within each child's record.
