Provided consultation on standard:
1) 350-Q: a center may temporarily reassign a child from his regular group and staff members for reasons of administrative necessity but not casually or repeatedly disrupt a child?s schedule and attachment to his staff members and group.
2) 370-6: stimulation and language development activities, including but not limited to staff reading, talking to, showing pictures to, naming objects for, playing with and engaging in positive interactions (such as smiling, cuddling and making eye contact) with infants
3) 570-H: formula or breast milk shall not remain unrefrigerated for more than two hours and may not be reheated.

Comments:
An unannounced complaint inspection was conducted with the local department of social services on 2/13/2023 to investigate an allegation received by the department regarding Special feeding needs. Additional interviews were conducted on 2/28/23. Based on evidence gathered through observations, interviews and documentation review, the complaint was determined to be valid. A violation notice was issued to the facility. An exit interview was conducted on 3/13/2023.

Description: Based on interviews and a review of documentation, the center failed to ensure that in each grouping of children at least one staff member who meets the qualifications of a program leader is regularly present. Evidence: Staff #1 and staff #2, who are regularly assigned to work together in the infant A classroom, do not have program leader qualifications. On 2/6/2023, staff #1 worked alone in the infant A classroom from approximately 7:00 ? 8:30am and from approximately 2:30 ? 4:00pm.

Complaint related: Yes
Description: Based on multiple interviews and a review of documentation, the facility failed to maintain a record accessible to parents and staff, that contained all required information. Evidence: there was no record for child #1 on 2/6/2023 indicating a) the amount of time the infant slept; b) the amount of food consumed and the time; c) a description and time of bowel movements; d) the amount of time spent on their stomach (tummy time.)

Complaint related: Yes
Description: Based on multiple interviews and a review of documentation, the center failed to ensure that infants were fed in accordance with parental instructions. Evidence: on 2/6/2023, staff were instructed during drop off to feed child #1 (age 10 weeks) every two hours, or earlier if needed. Child #1 arrived at approximately 7:20am, having last eaten at home at approximately 6:00am, and left at approximately 2:27pm. Six bottles with approximately three ounces each were provided, and at least four bottles were returned with approximately three ounces in each. There is no documentation that feedings, or attempted feedings, occurred every two hours as requested.
