Discussion on programs and supervision of children at all times.
Discussion on policies and procedures on staff reporting concerns and documenting staff concerns.

Comments:
A self report complaint was received in the licensing office on February 10, 2023 regarding a 18-month-old isolated from other children in the classroom and restrained in a chair. On February 14, 2023, telephone contact was made to the facility. On February 24, 2023, an on-site investigation was conducted. Interviews were conducted and observations were made.

Standard #: 8VAC20-780-340-A
Complaint related: No
Description: Based on staff interviews and documentation, the staff supervising children did not ensure their care, protection, and guidance.
Evidence:
1. During the course of an investigation, Licensing was made aware of an incident that occurred on or around March 31, 2022. Staff #1 left a child in the toddlers classroom unattended and went to another classroom. Child A walked out the classroom. It was undetermined how long Child A (two-year-old) was left unattended and unsupervised.
2. Child A was accompanied back to the classroom by Staff #1 when returned back to the classroom.
3. The Director was notified that Staff #1 left Child A in the classroom unsupervised.
4. Staff #1 acknowledge child was left unsupervised and unattended in the classroom.

Plan of Correction: This incident was not brought up to managements attention. It was only known and brought up until months after when an investigation took place.

Staff #1 was retrained and written up on a final as soon as management was made aware of the incident.

Management has retrained staff on supervision at the center. Children must never be left unattended. If another staff member sees lack of supervision from a peer they are mandated to notify management.

Description: Based on staff interviews and documentation, the staff supervising children did not ensure their care, protection, and guidance.
Evidence:
1. On February 9, 2023, Child B (18-month-old) was not attended to by Staff #2 in a nurturing and appropriate manner. Staff #2 did not include Child B in developmental learning activities with other children seated at the table in the classroom. Staff #2 did not engage in conversation with Child B during the activity.

Plan of Correction: All staff have be retrained on child supervision, discipline and behavior guidance. Staff member was placed on administrative leave.

Description: Based on staff interviews and documents, children under 10 years of age should always be within actual sight and sound supervision of staff.
Evidence:
1. On around March 31, 2022, Child A (2-year-old) was left the classroom unattended and walked out the classroom. It could not be determined how long the Child A was in the bathroom unattended and unsupervised.
2. Staff #1 acknowledge through written statements and interviews; a Child A was left in the classroom unattended.

Description: Based on staff interviews and documentation, the staff separated child from group so that the child is away from hearing and vision of a staff member.
Evidence:
1. Staff #2 separated Child B (18-month-old) from group of children in a strapped seated in chair and when in engaged in developmental learning activities.

Plan of Correction: All staff have been retrained on child supervision, discipline and behavioral guidance.
