An unannounced monitoring inspection was initiated and concluded on 3/16/2023. The inspector was on site from approximately 8:30 am-11:15 am. There were 55 children present, ranging in ages from 3 months to 5 years, with 10 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies, nutrition and background checks. A total of 5 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #: 8VAC20-780-150-A Description: Based on a review of five (5) child records and interview, the center did not ensure that the current form required by the Virginia Department of Health or a physician's form was used to report the immunizations received for one (1) child.
Evidence:
1. The record of child #5 (date of attendance: 2/6/2023) contained an immunization record that was emailed from the parent on 2/1/2023. The record was unofficial and generated from a patient portal.
2. Administration acknowledged that the record was not on the current form required by the Virginia Department of Health or an official physician's form.Plan of Correction: 2 office staff shall put eyes on folders and initial going forward-checklists will be utilized for compliance
Standard #: 8VAC20-780-280-B Description: Based on observation and interview, the center did not ensure that hazardous substances were kept in a locked place using a safe locking method that prevented access by children.
Evidence:
1. Two (2) cans of paint were observed in an unlocked lower cabinet in the center's second building. (activity center) The paint contained the words: warnings, do not ingest and avoid contact with skin and eyes. In a three year old classroom, an unlocked cabinet was observed that contained cleaning supplies.
2. Administration acknowledged that the hazardous substances were not locked.Plan of Correction: further staff training on 3/20/2023 will be done on locking of chemicals-administration will do random checks throughout weeks going forward
