22.1 Background Checks Code; carbon monoxide detectors

Comments:
A monitoring inspection was completed on Thursday, March 16, 2023 at the center. There were 77 children present with 19 staff supervising including administrative staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 6 child records and 4 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 22.1-289.058
Description: Based on observation and interview, the center did not ensure each building is equipped with at least one carbon monoxide detector.

Evidence: There was no carbon monoxide detector found at the center. Interview with the staff confirmed a carbon monoxide detector wasn't available for review during the inspection.

Plan of Correction: The center will order carbon monoxide detector today and install in building once received.

Standard #: 8VAC20-780-270-A
Description: Based on observation and photographs, the center did not ensure all areas and equipment of the center, inside and outside shall be maintained in a clean, safe and operable condition.

Evidence: There was observation of a blue rusted tricycle occupied by a child in care with both rubber handles missing and the seat cracked in multiple areas.

Plan of Correction: The center will throw equipment away.

Standard #: 8VAC20-780-290-A-3
Description: Based on observation, the center did not ensure all electrical outlets have protective covers.

Evidence: In the Lion classroom, there was observation of two electrical outlets uncovered and accessible to the children in care.

Plan of Correction: The center will add protective covers today.

Standard #: 8VAC20-780-500-A
Description: Based on observation and interview, the center did not ensure children's hands shall be washed with soap and running water after toileting and any contact with blood, feces, or urine.

Evidence: In Turtles classroom, there was observation of Child#1 diapering by staff and the child's hand were not washed after. Interview with staff confirmed Child#1's hand not being washed after diapering.

Plan of Correction: The center will retrain staff on correct handwashing procedures today.
