Comments:
An unannounced renewal inspection was conducted today from approximately 12:30pm - 1:45pm. A sample of 5 children?s records, and 5 staff records were reviewed. The children were observed napping, reading, and playing. Zero new staff have been hired since the last inspection. Areas of non-compliance are identified in this report. Standard #: 8VAC20-780-280-B
Description: Based on observation, hazardous substances such as cleaning materials were not kept in a locked place.

Evidence:
Cleaning supplies were located on a shelf in the bathroom of Room 116.

Plan of Correction: Cleaning materials will be locked in the cabinet.

Standard #: 8VAC20-780-550-P
Description: Based on record review, minor injury reports were missing required information.

Evidence:
Of 3 minor injury reports review, 2 were missing the time the parent was notified and 2 were missing the method in which the parents were notified.

Plan of Correction: Staff will be reminded to complete the entire form.
