Description: Based on review of the written attendance record, it was determined that the facility did not maintain a written record of daily attendance, for each group of children, that clearly and accurately documents the arrival and departure of children in care as it occurs. Evidence: 1) 4 children were observed in the infant A classroom, however only 3 children were included on the attendance sheet. 2) In the infant B classroom, 7 children were listed on the attendance sheet but only 4 children were observed. In both classrooms children were moved back and forth without clear and accurate documentation on the attendance sheet, indicating who was or was not in attendance in each classroom at any given time. 3) similarly, the attendance records for the other six classrooms also did not clearly state who was present and who was not based on the number of children who had moved from one class to another class, (and sometimes back again), and the children listed on the front and back of the attendance roster.

Description: Based on observation and a review of documentation, the center failed to ensure that in each grouping of children at least one staff member who meets the qualifications of a program leader is regularly present. Evidence: The records for Staff #1 and staff #2, who were observed working in the infant A classroom together, do not have documentation of program leader qualifications. The records for Staff #3 and staff #4, who were observed working in the toddler A classroom together, do not have documentation of program leader qualifications. The record for Staff #5, who was observed working alone in the two?s classroom, does not have documentation of program leader qualifications. The records for Staff #6 and staff #7, who were observed working in the preschool II A classroom together, do not have documentation of program leader qualifications. The records for Staff #8 and staff #9, who were observed working in the pre-K classroom together, do not have documentation of program leader qualifications.

Standard #: 8VAC20-780-350-Q
Description: Based on observation, staff interview and documentation review, it was determined that the center is casually reassigning children from their regular group for reasons other than administrative necessity disrupting the children's schedule and attachment to their regular group. Evidence: 1) The LI observed four crying infants being reassigned from the infant B classroom to the infant A classroom at approximately 9:50am when the second Infant A teacher arrived at the center. 2) Staff interviews revealed that children are reassigned regularly, whenever necessary to maintain ratios. 3) Approximately 24 children were moved on 2/28/2023 before 10:00am. Approximately six children were reassigned to a different classroom and then moved back to their original classroom, four less than 30 minutes later.

Standard #: 8VAC20-780-500-A
Description: Based on observation, not all handwashing procedures were followed, as required. Evidence: in the infant A and infant B classrooms, two children were observed drinking a bottle and their hands were not washed with soap and water before and afterwards. In both the infant A and Toddler classrooms, children were observed having a diaper change and the children's hands were not washed with soap and water afterwards.
