Standard #: 8VAC20-780-80-A
Description: Based on record review, the center did not maintain a written record of daily attendance for each group of children that documents the arrival and departure of each child in care as it occurs.

Evidence:
1. In the Fours room, there were 6 children present and 7 children were signed in on the attendance sheet.
2. In the Toddler room, there were 4 children present and 3 children were signed in on the attendance sheet.

Plan of Correction: Retrain on attendance tracking.

Standard #: 8VAC20-780-500-A
Description: Based on observation, the center did not ensure that children's hands were washed with soap and running water or disposable wipes before eating snacks.

Evidence: In the Threes room, the children were observed playing in centers and then cleaning up from centers when morning snack arrived to the classroom. The children were then seated at the tables and began eating morning snack which consisted of oranges and pretzels and did not wash their hands before beginning to eat.

Plan of Correction: Review proper hand washing procedure/time with staff.

Description: Based on observation, the center did not ensure that a nonabsorbent surface for diapering was used.

Evidence: The changing pad in the Twos room had approximately a 1" tear in the material exposing the foam filling making the changing pad absorbent.

Plan of Correction: New changing pad ordered and replaced.

Standard #: 8VAC20-780-570-E
Description: Based on observation, the center did not ensure that prepared infant formula was dated and labeled with the child's name.

Evidence: In the Infant room, there was one bottle of prepared infant formula in the refrigerator that was not dated or labeled with the child's name.

Plan of Correction: Remind staff bottles need to be labeled when received.
