Comments:
An unannounced monitoring visit and facility tour was conducted on 3/14/2023. At the time of inspectors' arrival there were 46 school aged children in care with 4 staff members. Children were observed interacting with staff, free play, snack, handwashing, and departure. A sample of 5 children's records and 4 staff records were reviewed. Injury reports and medications were also reviewed. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Violations:
Standard #: 8VAC20-780-60-A
Description: Based on record review and interview, the center failed to maintain and keep at the center a separate record for each child enrolled.

Evidence:
1. Child #2, enrollment date 9/6/22, lacks an enrollment record containing the following information:
- Home address and home phone number of each parent;
- Work phone number and place of employment of each parent;
- Name, address and phone number of two designated people to call in an emergency;
- Names of persons authorized to pick up the child;
- Previous child day care and schools attended.
2. Staff confirmed this information was not available for child # 2.

Plan of Correction: Staff will request information from corporate office and/or parents.

Standard #: 8VAC20-780-70
Description: Based on record review and interview it was determined that the center did not ensure that each staff record contained the required components.
Evidence:
1. The staff record for staff #4 (hired 3/15/21) did not contain the name, address, and phone number of an emergency contact.
2. Staff confirmed this information was not available during this inspection.

Plan of Correction: Staff will provide emergency contact information.

Standard #: 8VAC20-780-240-A
Description: Based on record review and interview it was determined the center did not ensure that each staff member completed the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.
Evidence:
1. The records for staff #1(hired 9/6/22) and staff #3(hired 9/21/22) did not contain documentation of completion of the Virginia Department of Education-sponsored orientation course.
2. Staff confirmed this documentation was not available.

Plan of Correction: Staff will complete and/or provide documentation of completion of required orientation training.

Standard #: 8VAC20-780-540-C
Description: Based on observation and interview it was determined the center did not ensure that the first aid kit contained all of the required components.
Evidence:
1. There was no tweezers or ice packs in the first aid kit.
2. Staff confirmed that these items weren't available during this inspection.

Plan of Correction: Staff will request the needed first aid supplies.

Standard #: 8VAC20-780-550-P
Description: Based on record review and interview it was determined that the center did not ensure the written record of children's serious and minor injuries contained all of the required information.
Evidence:
1. The injury report for child #6 dated 2/22/23 did not contain the date and time of parental notification and was missing one of the two required signatures.
2. Staff confirmed this information was not on the report.

Plan of Correction: Staff will be retrained on documenting injuries.
