An unannounced on-site monitoring inspection was conducted on 03/22/2023 and a virtual staff record review was conducted on 03/24/2023. There were 25 children, ages 6 years-11 years, and 4 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, programs, emergencies and nutrition. The children were observed arriving to the program, during large motor play, and having an afternoon snack. A total of 5 children?s records, 4 staff records and 1 medication were reviewed. The inspector arrived to the center at 2:35 pm and departed at 4:00 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard #: 8VAC20-780-140-A Description: Based on record review, the center failed to obtain documentation that each child shall have a physical examination by or under the direction of a physician before the child?s attendance or within 30 days after the first day of attendance.
Evidence:
1. The record for Child 1 contained a documented start date of 10/14/2022. There was not documentation of a physical examination in the record for Child 1.
2. The record for Child 2 contained a documented start date of 09/06/2022. There was not documentation of a physical examination in the record for Child 2.
3. The record for Child 4 contained a documented start date of 08/01/2022. There was not documentation of a physical examination in the record for Child 4.Plan of Correction: Will get the information and check all records.
Standard #: 8VAC20-780-60-A Description: Based on record review, the center failed to ensure that the separate record for each enrolled child contains all of the elements as required by the standards.
Evidence:
1. The record for Child 3 did not contain the name, address and phone number of two designated people to call in an emergency if a parent cannot be reached (the address for the second designated person was incomplete).
2. The record for Child 4 did not contain the name, address and phone number of two designated people to call in an emergency if a parent cannot be reached (there was no address listed for the first designated person and the address for the second designated person was incomplete).Plan of Correction: Will get the information and check all records.
