Comments:
A monitoring/renewal inspection was initiated and concluded on February 23, 2023. There were 39 children present ages two through five years old with eight staff and one administrator. During the inspection a walking tour of the center inside and out was completed. A sampling of children and staff records were reviewed. Children were observed in teacher and self-directed activities including outside play, lunch, transitions, hand washing and rest period. Additionally, the fire, health inspection, emergency equipment and emergency evacuation drills were reviewed to determine if required documentation and equipment were in compliance with standards. Violations regarding program leader qualifications, unlocked chemicals, accessible purses, accessible diaper cream and plastic bags were observed and discussed with the program director during the exit interview at the conclusion of the inspection.

Evidence:
1 - The record for Staff #2 did not contain documentation of education. The staff was working with children in the role of program leader.
2 - Staff #1 confirmed that the required documentation was not present in the staff record.

Description: Based on observation the center failed to ensure that areas are maintained in safe condition.
Evidence:
There are plastic bags and dry beans accessible in the two year old room. These items pose choking and suffocation risks to children

Description: Based on observation and interview the center failed to ensure that hazardous substances are kept in a locked place.
Evidence:
1 - In the adult bathroom on the first floor there are a spray can of air freshener and a spray can of glass cleaner stored in a cabinet over the toilet without any type of lock or child proof latch.
2 - Staff #1 stated that the cabinet had been replaced and a new latch had not been put on the cabinet.

Evidence:
In the two year old classroom there is diaper ointment and cream stored in plastic bins on shelves within reach of children.
