Standard #: 8VAC20-790-540-B-3 Description: Based on record review, one of two children's records did not have the address for each custodial parent's place of employment or school attendance.
Evidence:
1. Child #2 had the parent's place of employment and one work phone number, but did not have the address of the custodial parent's place of employment or school attendance.Plan of Correction: We will use the information we have elsewhere on file to fill in the necessary parent information.
Standard #: 8VAC20-790-600-D-1 Description: Based on review of training, one staff member who works directly with children was not current in cardiopulmonary resuscitation (CPR) within 90 days of the date of
employment.

1. Staff C (start date 10/17/2023) did not have certification of CPR.Plan of Correction: The staff member in question will be set up to take her CPR training in the April cohort.
