Comments:
An unannounced monitoring inspection was conducted on 3/16/2023 from 3:00 pm to 5:00 pm. There were 12 children in care, ages 5 years to 12 years in one group under the care of two staff. The Licensing Inspector reviewed five children?s records and four staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. The children were observed washing hands, having snack, and indoor play.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Description: REPEAT VIOLATION

Based on record review, the center failed to obtain documentation of child updates and confirmation of up-to-date information in the child?s record.

EVIDENCE:
1. The record for child 2 has not been updated since 6/1/2021.

2. The record for child 3 has not been updated since 9/7/2021.
